Look at the sample forms at http://money.howstuffworks.com/forms-samples.htm and see if that's what you are looking for.
To be honest, I usually just make up my own using a ruler and lined paper. Sometimes I pick up a ledger at Walmart or an office supply store.

If you are using MS Office, there are some balance sheet templates in the MS website.
